The song is "On the Sunny Side of the Street". I'm not sure on the pianist, possibly Oscar Peterson, Teddy Wilson or another early bop or swing era pianist with very clean technique, but definitely not Theolonious Monk.
Try going on youtube and put 'pianist playing on the sunny side of the street'  or 'piano trio playing on the sunny side of the street' and see what comes up.

I just checked out some versions, Teddy Wilson, George Shearing and Oscar Peterson usually play it slower than your version, but Erroll Garner, Earl Hines, Duke Ellington and Art Tatum play it at a tempo closer to your version.

Here is Art Tatum, its somewhat close to your version.
